Patient Notification Menu - Emergency Contact Information - Cal-PM
Manage emergency contact client information.
- Multiple client records can be maintained.
- This form is for information-only and can be modified using the modeling functionality of RADplus Utilities.
- Go to: Avatar PM > Client Management > Client Information > Patient Notification Menu
- In the Select Client screen, enter the client name or ID in the Client Name/ID field, select the Search button, and select the corresponding entry.
- If the client has multiple episodes, the Episode Selection pre-display shows. Select the episode and then OK.
- If the client has previous Emergency Contact Information records, select the record. Select Edit to change information, or Add to create a new record.
- In the Emergency Contact Name field, enter the emergency contact’s name in the format LAST,FIRST.
- In the Emergency Contact Relationship field, select the contact's relationship to the client.
- For the Emergency Contact Living With Client field, select Yes if the client is living with the contact.
- In the Emergency Contact demographic fields, enter the contact's demographic information.
- Select Submit.
