Patient Notification Menu - Emergency Contact Information - Cal-PM

Manage emergency contact client information.

  • Multiple client records can be maintained.
  • This form is for information-only and can be modified using the modeling functionality of RADplus Utilities.

  1. Go to: Avatar PM > Client Management > Client Information > Patient Notification Menu
  2. In the Select Client screen, enter the client name or ID in the Client Name/ID field, select the Search button, and select the corresponding entry.
    • If the client has multiple episodes, the Episode Selection pre-display shows. Select the episode and then OK.
    • If the client has previous Emergency Contact Information records, select the record. Select Edit to change information, or Add to create a new record.
  3. In the Emergency Contact Name field, enter the emergency contact’s name in the format LAST,FIRST.
  4. In the Emergency Contact Relationship field, select the contact's relationship to the client.
  5. For the Emergency Contact Living With Client field, select Yes if the client is living with the contact.
  6. In the Emergency Contact demographic fields, enter the contact's demographic information.
  7. Select Submit.
