Integrated eSignature documents for myAvatar
Using the Integrated eSignature process, myHealthPointe allows for electronically signed documents to be captured and saved to both the Consumer and Provider portals directly from myAvatar NX.
- Documents can be sent from within myAvatar NX to myHealthPointe for electronic signature or for view only.
- When a document is signed in myHealthPointe, the document is automatically returned to myAvatar NX.
- Case use examples for this functionality consist of consent forms, treatment plans, and custom client-based entity modeled forms.
- myAvatar NX allows for document routing for signatures prior to sending to a client in myHealthPointe and after the document is signed by the client.

Prerequisites:
- myAvatar NX must be configured to integrate with myHealthPointe.
- A client is enrolled in an existing episode with the following on file:
- Date of Birth
- Email Address
- Login credentials for myHealthPointe
- The Enable Send Document to myHealthPointe functionality registry setting is set to Y.
- Document Routing is enabled on the Ambulatory Progress Notes form and an approver is not required.
- The Pending eSignatures widget must be accessible on the HomeView in myAvatar NX.
- Search for the Ambulatory Progress Notes form or go to
- Select the client.
- Select Independent Note in the Progress Note For field.
- Select the desired value in the Note Type field.
- Enter the desired value in the Notes Field field.
- Select Final in the Draft/Final field.
- Click Submit.
- Select Accept and Route in the Confirm Document dialog box.
- Enter the password for the logged in user in the Password field and click Verify.
- Select Collect eSignature in the Send to myHealthPointe field.
- Click Submit.
System Results:
- The eSignature request displays in the Pending eSignatures widget in myAvatar NX.
- The eSignature request displays with a status of Pending on any report using the DocR.esignature SQL table in myAvatar NX.
- The eSignature request displays in the Patient Documents list and the Request Signature tab in the myHealthPointe Provider Portal.
- Patient logs in to the Consumer Portal.
- In the Home menu on the left, the patient selects Documents Awaiting Signature.
- Documents that require signature are visible in the applet.
- The patient selects Sign and Submit to view the document and sign.
- Documents can be downloaded or printed from the Enter Signature view.
- Fillable PDFs can be completed at this time.
- The patient logs in to the Netsmart myHealthPointe application on their mobile device.
- On the home page, the patient selects Unsigned Consent Forms to review a list of documents requiring signature.
- Once the Consent Form is selected, the document opens for review and signature by the patient.
System Results:
- Once the document is signed by the patient from within the Consumer Portal, the document displays in the Documents Awaiting Signature list within the myHealthPointe Provider Portal with a green checkmark in the Signed column and a timestamp in the Date Signed column.
- The eSignature request no longer displays in the Pending eSignatures widget in myAvatar NX.
- The eSignature request displays with a status of Approved on any report using the DocR.esignature SQL table in myAvatar NX.
- Two document rows display for the client in the Clinical Document Viewer form.
- One document description contains Integrated eSignature Request - this is what was sent to the myHealthPointe Consumer Portal for eSignature. The document displays the author's signature and the patient's signature appended to the bottom right corner.
- One document description contains Ambulatory Progress Note - this is the progress note finalized via document routing. The document displays the author's signature and the patient's signature appended to the bottom right corner.

- Enter the password for the logged in user in the Password field and click Verify.
- Select Document Only in the Send to myHealthPointe field.

- Populate all required and desired fields.
- Select Final in the Treatment Plan Status field.
- Click Submit.
- Select Accept and Route in the Confirm Document dialog box.
- Enter the password for the logged in user in the Password field and click Verify.
- Select Collect eSignature in the Send to myHealthPointe field.
- Click Submit.
System Results:

- In the Form Type field, select the note type associated to the Progress Notes (Group and Individual) form previously created.
- Select the client in the Entity field.
- Enter the current date in the From Date and To Date fields.
- Select the progress note filed in the previous steps from the List of Documents field.
- To confirm the client signed the document, select Display Document.
- The document displays with the client signature appended to the bottom right corner.
- Select Close All Documents and Exit.
- In the New Comments to Be Appended to the Original Document field, enter the new information.
- Click Submit.
- Select Accept and Route in the Confirm Document dialog box.
- Enter the password for the logged in user in the Password field and click Verify.
