Reason for Visit - STD module
In the STD module, the Reason for Visit form is where you document reason(s) for the patient's visit, such as vaccines required or treatment for positive tests. Also use this form to record the patient's referral contacts and any reexaminations or rescreenings that are needed.
On the main menu click Reason for Visit.
Record the reason for visit
- In the Reason for Visit panel, select the choice that best describes the purpose of the visit.
- If Partner diagnosed with STD is selected, in the Partner Diagnosis box, enter a description.
- If Test only is selected, the patient is required to respond to the following items.
- Patient understands and accepts a test only visit - Select Yes. This is required.
- Patient confirms understanding that a test only visit does not include a physical exam - Select Yes. This is required.
- For the Had oral, anal, or vaginal sex with HIV infected partner in the past 72 hours item, select Yes if applicable.
- In the Reexam/Rescreen For panel, select Yes for each exam or screening to be conducted.
- If Epididymitis is selected, in the Referring Provider box, enter the provider's name in the box provided.
- If Other is selected, enter a description in the box provided.
- In the Diagnosed With panel, select all diagnoses that apply to the patient.
- In the Vaccine(s) Required During Current Visit panel, select all vaccines required.
- In the Tx For + Test panel, provide the following information.
- For the Syphilis Test, HSV Test, and Trichomonas Test items, select Yes for each test the patient will receive treatment for.
- In the Other + Tests Received box, enter a description for other positive positive tests the patient has had.
- Under CT Site and GC Site, select the check boxes for items that apply to the patient.
- In the CDI Referral - Contact panel, provide the following information.
- In the CDI Referral - Contact CDI Name (If Known) field, click the arrow and select the contact.
- In the CDI or Provider Referral - Disease Referred By (If Known) field, click the arrow and select the contact.
- For the Field Record item, select Yes if this item applies to the patient.
- Under Contact To, select the check box for each item that applies to the patient.
- In the Last Exposure Date field, enter the date. Optionally click the previous and next arrows or the calendar icon
to select a date.
- On the toolbar above the menu, click the Save icon
