History of Syncope - STD module

In the STD module, the History of Syncope form is where you document the patient's history of loss of consciousness.

On the main menu navigate to History > History of Syncope.

Review history

If you reviewed the syncope history with the patient, for the Syncope History Reviewed item at the top of the form, select Yes.

Add syncope history

  1. Below the Syncope History panel, click Add Entry.
  2. In the Prior Syncope Event box, click the arrow and select the appropriate choice. If Other is selected, enter details in the box provided.
  3. In the Notes box, enter additional information.

Note: To clear the entry and start over, click Reset. To discard the entry, click Cancel.

  1. Click Save Entry.

Edit syncope history

  1. In the grid, click the pencil icon  next to the entry you want to edit.
  2. Change the information and click Update Entry.
