Care Plan - STD module
In the STD module, the Care Plan form is where you record events and information regarding the patient's current visit and items to be handled post-visit. Included with this form is an Encounter Summary that you can print for the patient.
On the main menu click Care Plan.
Create a care plan
- In the Lab Results Provision panel, for each type of result provided during the visit, select the Yes check box.
- In the Field CDI Details panel, select the Yes check box if a CDI interview needs to be done with the patient.
- In the Patient Follow Up panel, provide responses for the following items.
- Under Reason For Follow Up, select the check box for each reason that applies to the patient. If Other is selected, enter details in the box provided.
- In the Reason For Follow Up - Description box, enter a description of the reason.
- In the Return to Clinic On box, enter the date for the patient's follow-up appointment. Optionally click the previous and next arrows or the calendar icon
to select a date. To clear the date, click the X.
- Under Referrals Provided, select the check box for each type of referral provided during the visit.
- Under Wait on premises until, select the time that the patient is to wait until before leaving the premises. Either click the arrows to select the time or click the clock icon
to insert the current time. To clear the time fields, click the X.
- For the Patient remained on site post treatment and Patient left AMA items, select Yes if applicable.
- For the Discharge time, either click the arrows to select the time or click the clock icon
to insert the current time. To clear the time fields, click the X.
Print instructional documents
The Instructional Document Print Station panel is where you select and print the patient's instructional and educational documents for the following categories.
- Vaccine Information Sheets
- At Home Treatments
- In Clinic Instructions
- Disease-Specific Educational Materials
- Patient Delivered Partner Treatment
- In the Instructional Document Print Station panel, click the blue arrow for the type of document you want to print.
- In the list of available documents, click a document to select it. The row will turn green.
- In the Print column, click the arrow. The document opens in a new browser tab.
- Close the tab to return to the document list.
Print a single document
- In the row for a category, click the blue downward arrow.
- In the drop-down list, click a row to select the document. The row will turn green.
- Click Print. The document opens in a new browser tab.
- In the toolbar above and to the right of the document, click the printer icon.
- In the Print window, select the options and click Print.
- Close the browser tab that shows the document.
Print multiple documents
- In the row for a category, click the blue downward arrow.
- In the drop-down list, click a row to select a document. The row will turn green.
- When you finish selecting documents in the category, click Done. Repeat this process to select documents in this category and other categories.
- For each category, a document icon appears that shows the number of documents in that category that are queued for printing.
- Below the grid, the Print xx Documents button shows the total number of documents in all categories that are queued for printing.
- Click the Print xx Documents button.
Encounter summary
The Encounter Summary provides a snapshot of the patient's visit that includes the following information.
- List of medication, lab, and vaccine orders generated or discussed during the encounter
- Staff person's notes about the encounter
- Contact information for the patient's doctor and their provider
To print the Encounter Summary, click Print Visit Summary at the bottom of the form.
