Abuse Assessment

In the STD module, the Abuse Assessment form is where you document the patient's history of intimate partner violence (IPV) and notifications to law enforcement regarding abuse. 

  1. Go to: History > Abuse Assessment.
  2. Below the IPV History panel, click Add Entry.
  3. The Date Recorded defaults to today's date and cannot be changed.
  4. In the IPV Event box, click the arrow and select the appropriate choice. If Other is selected, enter details in the box provided.
  5. In the Status box, click the arrow and select the appropriate choice.
  6. In the Notes box, enter additional details.
    Note: To clear the entry and start over, click Reset. To discard the entry, click Cancel.
  7. Click Save Entry.  

Law enforcement

For each of the following items, select Yes if the statement applies to the patient's situation.

  • Patient notified law enforcement will be contacted
  • Law enforcement notified by phone
  • Law enforcement notified by fax
  • Law enforcement onsite

Add form faxed to law enforcement

  1. Below the Form Faxed to Law Enforcement panel, click Add Entry.
  2. In the Form Faxed box, click the arrow and select the appropriate choice. If Other is selected, enter details in the box provided.
  3. In the Form Scanned to EHR box, click the arrow and select Yes or No as appropriate.
  4. In the Date Faxed box, enter the date. Optionally click the previous or next arrows or the calendar icon to select a date. To clear the date, click the X.
  5. Note:  To clear the entry and start over, click Reset. To discard the entry, click Cancel.
  6. Click Save Entry.
