Abuse Assessment
In the STD module, the Abuse Assessment form is where you document the patient's history of intimate partner violence (IPV) and notifications to law enforcement regarding abuse.
- Go to: History > Abuse Assessment.
- Below the IPV History panel, click Add Entry.
- The Date Recorded defaults to today's date and cannot be changed.
- In the IPV Event box, click the arrow and select the appropriate choice. If Other is selected, enter details in the box provided.
- In the Status box, click the arrow and select the appropriate choice.
- In the Notes box, enter additional details.
Note: To clear the entry and start over, click Reset. To discard the entry, click Cancel. - Click Save Entry.
Law enforcement
For each of the following items, select Yes if the statement applies to the patient's situation.
- Patient notified law enforcement will be contacted
- Law enforcement notified by phone
- Law enforcement notified by fax
- Law enforcement onsite
Add form faxed to law enforcement
- Below the Form Faxed to Law Enforcement panel, click Add Entry.
- In the Form Faxed box, click the arrow and select the appropriate choice. If Other is selected, enter details in the box provided.
- In the Form Scanned to EHR box, click the arrow and select Yes or No as appropriate.
- In the Date Faxed box, enter the date. Optionally click the previous or next arrows or the calendar icon to select a date. To clear the date, click the X.
- Note: To clear the entry and start over, click Reset. To discard the entry, click Cancel.
- Click Save Entry.
