Skip to main content
- Select the type of diagnosis.
- Enter the diagnosis date.
- Enter the diagnosis time in the format HH:MM AM/PM.
- In the Diagnosis Axis fields:
- Type the diagnosis, select Enter. Choose the diagnosis from the list.
- Diagnosis axis codes are defined in the Diagnosis Table Maintenance form in RADplus Utilities.
- Select the principal diagnosis. The list contains diagnoses codes entered in Axis I and II fields.
- Select the diagnosing practitioner.
- Enter any additional information as needed.
