Substance Abuse History - PM

Maintain substance abuse information for a client.

• This form is for information-only and can be modified using the modeling functionality of RADplus Utilities.

1. Go to: Avatar PM > Client Management > Client Information > Substance Abuse History.
2. In the Select Client screen, enter the client name or ID, and select. Click Select.
   • If the client has multiple episodes, the Episode Information pre-display shows. Select the episode and click Ok.
   • If the client has previous substance abuse history records, select the record. Click Edit to change information, or click Add to create a new record.
3. In the Drug Of Choice fields, select the drug used by the client.
4. In the Usual Route Of Administration fields, select the way in which the drug is used.
5. In the'Frequency Of Use fields, select how often the drug is used.
6. In the Age At 1st Use/Intoxication field, enter the age of the client when the drug was first used.
7. In the Current Amount fields, enter the amount of the drug currently being used by the client.
8. In the AA/NA Involvement field, select the client's participation in AA and/or NA.
9. In the Longest Sobriety field, select the client's longest length of sobriety.
10. Click Submit.
