Practitioner Inquiry - PM

Generate a report on information for any myAvatar practitioner-related form.

Prerequisite:

  • A practitioner must be enrolled in myAvatar through the Practitioner Enrollment form.
  1. Go to: Avatar PM > Practitioner > Practitioner Information.
  2. In the Practitioner Option field, select the practitioner form to generate the report. This list displays all myAvatar practitioner forms the user has access to.
  3. In the Individual Or All Practitioners field, select to generate a report including all practitioners, or an individual practitioner.
    • If Individual is selected, enter the practitioner name or ID in the Practitioner field and select.
  4. In the Inquiry Start Date field, enter the report start date.
    • The start date should be no later the practitioner’s enrollment date.
    • If the start date is unknown, or if a system-wide inquiry is required, it is recommended that the “go-live date” be used.
  5. In the Inquiry End Date field, enter the report end date.
  6. Select Process.
