Patient Notification Menu - Patient Notification - PM

Maintain patient contacts that can be notified on behalf of the client.

  • This form is for information-only, and can be modified using the modeling functionality of RADplus Utilities.

  1. Go to: Avatar PM > Client Management > Client Information > Patient Notification Menu > Patient Notification.
  2. In the Select Client screen, enter the client ID in the Client Name/ID field, select the Search button, and select the corresponding entry.
    • If the client has multiple episodes, the Episode Information pre-display shows. Select the episode and then, OK.
    • If the client has previous patient notification records, select the record. Select Edit to change information, or Add to create a new record.
  3. In the Person To Notify field, enter the contact name using the format LAST,FIRST.
  4. In the Person’s Relationship To Patient field, select the person.
  5. In the Person To Notify fields, enter the person's demographic information.
  6. In the Notified field, select Yes if the person was notified.
  7. In the Date Notified field, enter the notification date.
  8. In the Time Notified field, enter the notification time.
  9. In the Notified By field, enter the person who notified the contact in the format LAST,FIRST.
  10. Select Submit.
