Manage practitioner information (Brief) - PM

Practitioner Enrollment (Brief) allows you to manage basic practitioner information.

  • Editing data for a practitioner does not edit the practitioner’s historical information. For example, if a practitioner was originally enrolled as a nurse, and has changed their designation to a nurse practitioner, modifying the practitioner’s title does not change the date of enrollment. It will appear as though the practitioner was a nurse practitioner since the original enrollment date. It is recommended that the original entry be deleted (terminated) and a new practitioner ID be created.
  • Some of a practitioner's demographic information can be updated after termination. To edit the remaining fields, the practitioner needs to be re-enrolled.

  1. Go to: myAvatar PM > Practitioner > Practitioner Registration > Practitioner Enrollment.
  2. In the Select Staff screen, enter the staff name or ID, and Select.
    Note: If the staff member is not found, a dialog box displays. Select New Staff
    Result: The Auto Assign dialog displays.
  3. Select Yes to generate a staff ID number
    Or
    Select No to enter an ID number.
  4. In the Name field, enter the practitioner name using the format LAST,FIRST.
  5. In the Registration Date field, enter the practitioner’s employment start date. 
    Note: The employment date cannot be changed after this form is submitted.
  6. In the Date Of Birth field, enter the practitioner’s birth date.
  7. In the Practitioner Type field, use the dropdown menu to make a selection. 
  8. Enter the address and phone number information for the practitioner's Office and Home.
  9. In the Sex field, use the radio buttons to indicate the sex of the practitioner.  
  10. n the Ethnic Origin drop-down menu to select.
  11. In the Beeper Number field, enter the practitioner's beeper number, if applicable.
  12. In the NPI Number field, enter the NPI number for the practitioner.  
  13. In the Tax Identification Number field, enter information.
  14. In the Performing Provider field, enter the provider ID number in the search field and make a selection.  
  15. In the Performing Provider Registration field, use the dropdown menu to make a selection. 
  16. In the Secondary Provider Identification Number field, enter relevant information. 
  17. In the Fax Number field, enter the fax number for the practitioner.  
  18. In the Staff EVV ID field, enter the ID for the practitioner.  
  19. In the Office Location ID field, enter the ID for the location.  
  20. In the Email Address field, enter the email address for the practitioner.  
  21. In the Staff EVV Type field, enter relevant information. 
  22. When finished, select Submit.
