Granting Consent Within myAvatar

Step 1: Consent for Access Form

  • Select the Consent for Access Form.  If test is not already selected, you will be required to specify client prior to loading the form.   Note:  If referrals have not been configured, the user will receive a message indicating that the referral provider directory is out of date and ask if you want to continue.  Choose Yes to Continue. ​

Step 2: 42 CPFR Part 2 - First section within the form

  • Yes = Client is enrolled in a program that involves substance abuse education, treatment, or prevention.
  •  Workflow note:  As part of any admission into a new program, the organization should be sure to review the Consent For Access form to determine whether or not the program they were admitted to meets 42 CFR Part 2 Regulations.  If they do not and the client is initially not admitted to a program that meets these requirements, client information may get uploaded inappropriately if they are later admitted to a program that does. 
  • No = Client is not enrolled in a program that is covered under 42 CFR. 
  • Not Indicated = No indication has been made as to whether or not the client is covered under 42 CFR.  

Step 3: HIE Section - Second section within form

  • Choose to Add new Item
  • HIE to Access = Able to select from a list of HIE’s configured as part of the CCD Upload Configuration Form.  Note:  If you cannot find the HIE configured in previous steps – return to the Configuration Form and ‘Refresh HIE List’
  • Consent to Query
    • Grant = Client has granted consent for organization to access their health information within the HIE
    • Revoke = Client has revoked consent for organization to access their health information within the HIE
    • Emergency Only = Client has granted emergency consent to the organization for access to their health information.  Please note  - in this scenario consent may or may not be accessible via myAvatar.
    • If the HIE has been configured to require consent on send, then this field and associated options will be disabled
  • Consent to Send
    • Grant = Client has granted consent for organization to send their health information to the HIE
    • Revoke = Client has revoked consent for organization to access their health information within the HIE
    • There is no Emergency Only Option for Consent to Send.
    • If the HIE has been configured to require consent on query, then this field and associated options will be disabled.
  • Start Date = Date Consent was granted
  • End Date = Date Consent ends.  Workflow note:  Some organizations may have a policy in place where by consent must be renewed each year.  In that scenario they may choose to always enter an end date when granting consent, though an end date is not required.  In the instance no end date is required; consent will remain in effect until revoked. 
    • If consent is being revoked, end date will not be enabled.  

Once filed, individual rows can be edited, but not deleted.  Simply select the row and choose the appropriate option.  

If the registry setting requiring individual agency consent has been enabled, an additional field will be available in the form labeled agencies specified.  This allows the user to configure all providers for an individual agency to query for information.  This does not impact whether or not the CCD is uploaded to the HIE.  The dictionary is user defined to be populated with agencies and has an extended dictionary element to specify the agencies organization id.

Submit the form.

Congratulations – the client has granted consent for sending/receiving information!
