Client Observation

Record observation results collected throughout the day.

  1. Go to Avatar CWS > Assessments > Product Assessments.
  2. In the Select Client screen, enter the client ID in the Client Name/ID field, search for and select the client.
    • If the client has multiple episodes, select the client's episode, then choose Ok.
  3. In the Observation Date field, enter the date for the client observation.
  4. The Currently Assigned Observer field displays the user currently who has the selected client in their caseload.
  5. Select Standard Duration if the behavior's duration is standard.
    • In the Time field, enter the time this observation was assigned.
  6. Select 1:1 Routine if the observation was routine.
    • In the Time field, enter the time this observation was assigned.
  7. Select 1:1 Two Arms Length if the 1:1 Two Arms Length observation was assigned to the user.
    • In the Time field, enter the time this observation was assigned.
  8. In the Reason for Precaution field, select the behavioral issues the client might be presenting.
  9. In the Other Reason for Precaution field, enter precautions not listed in the Reason For Precaution field.
  10. In the Observation Summary field, enter summary notes for the observation.
  11. Select Submit.
