Functional Assessment

Record a functional assessment for a member.

  • This assessment works with the Care Notes and Documentation form, and contains information displayed in the Functional Assessment report.

  • This is a user-defined form.

  1. In the Select Client screen, search for and select the client by name or ID.

  • If the client has previous functional assessments, the pre-display displays.
  • Select a functional assessment to edit.
  1. In the Date of Assessment field, enter the assessment date.

  2. In the Assessment Type field, select assessment type.

  3. Enter additional functional assessment information as appropriate.

  4. Click Submit.

  • The Functional Assessment Report is generated, which displays the client's functional assessment information, and scores.
