Accreditation Section

Enter accreditation information for a contracting provider.

1. In the 'Reviewed' fields, select Yes if the associated information was reviewed.

2. In the Is CARF Accreditation Letter Attached field, select Yes if the letter is attached.

3. In the Other Accreditations field, select additional accreditations.

4. In the Facility Medicare Number field, enter the number.

5. In the Facility Medicaid Number field, enter the number.
