Health History Section

Enter the foster home member's health history.

1. The Foster Home Member Health History table lists all of the existing health history records on file for this individual. Click Add New Item to enter a new record.
2. In the Condition Reported field, enter the name or description of the health condition.
3. In the Treatment Start and End Date fields, enter the date range during which the foster home member was treated for this condition.
4. In the Medication(s) For Reported Conditions field, enter the names and details for any medications the foster home member was taking for this condition.
5. In the Doctor's Name field, enter the name of the physician overseeing care for this condition.
6. In the Doctor's Address field, enter the address information this physician.
