Foster Home Registration

Record and track an active Foster Home participating in the Foster Care Program including requirements, the types of placements they accommodate, and whether or not they are accepting placements.

Prerequisites:


  1. Go to: Avatar MSO > Provider Management > Foster Home Registration
  2. In the Select Provider menu, search for and select an existing foster home applicant to register as an official foster home.
  3. The Foster Home Name field displays the name entered on the Foster Home Initial Registration form, but can be edited if necessary.
  4. The Foster Home Registration Date field identifies the date on which this foster home was official registered for use within the Foster Care Program.
  5. The Eligible Funding Source field defaults based on the entry in the Default Funding Source field of the Foster Care Defaults form. If no entry defaults, the source of reimbursement can be entered here.
  6. In the Maximum Bed Capacity field, enter the number of beds which the foster home has available for placements.
  7. In the Home Phone fields, enter the phone numbers associated with this foster home.
  8. In the Make Checks Payable To field, enter the name of the person or foster home to whom checks should be addressed.
  9. In the Placement Type Accepted field, select the types of placements that are accepted by this foster home.
  10. In the Current Placement Status field, select whether or not the home is currently accepting new placements from the Foster Care Program.
  11. The Home Address fields defaults the physical address of the foster home from the Foster Home Initial Registration.
    The County can be updated, as necessary.
  12. In the Primary Mailing Address fields, enter the address to which mail should be sent.
  13. In the State Assigned ID Number field, enter the ID Number assigned to this Foster Home by the state.
  14. In the Administrative Hold Start Date field, enter the start date of the administrative hold. This field is enable and required when Administrative Hold is selected in the Current Placement status field. 
  15. In the Administrative Hold End Date field, enter the end date of the administrative hold. This field is enable and required when Administrative Hold is selected in the Current Placement status field.
  16. In the Administrative Hold Reason field, select a hold reason from the dropdown.   This field is enable and required when Administrative Hold is selected in the Current Placement status field. Administrative Hold Reason is set up in the MSO Dictionary Update form (Provider File – Data Element 528). 
  17. Click Submit.
