Foster Child Behaviors

Record and track a foster child's behaviors to help ensure appropriate foster home placement.

This form is not episode-specific.
Via Crystal Reporting, data entered here will be compared to similar data at the Foster Home Level to help identify an appropriate foster home for the foster child based on the home's ability to meet the needs of the child's behaviors.

Go to: Avatar MSO > Membership Management > Foster Child Behaviors
On the Select Client screen, search for and select a foster child.
In the Level Of Care field, select the intensity of care needed by this child.
Select Yes or No the following questions:
- Is The Child Part Of A Sibling Group?
- Has The Child Been A Victim Of Sexual Abuse?
- Does The Child Have Parents/Caregivers With A History Of Violence?
- Has The Child Had Multiple Short Term Placements?
- If The Child Is Female, Is She Currently Pregnant Or Does She Have A Child?
- If The Child Is Male, Does He Have Custody Of Any Children?
In the School District field, select the child's school district.
In the Ethnic Origin field, select the child's ethnicity.
When finished, click Submit.
