About assessment history in Flowsheet

In myAvatar Flowsheet, select the clock icon above any committed or signed assessment to view the assessment history. History data can include the following items:

Created assessment

• Assessment data with the date/time created
• First Name, Last Name, and Credentials of the person who signed the assessment
• Assessment fields and values

Edited assessment

• Edited assessment data with the date/time edited
• First Name, Last Name, and Credentials of the person who edited the assessment
• Reason for edit
• Assessment fields, previous values, and new values

Voided assessment

• Assessment voided date/time
• First Name, Last Name, and Credentials of the person who voided the assessment
• Reason for void
