Enter information about the client's living arrangements.


  1. Go to: Avatar PM > Client Management > California Data Reporting > MHSA Assessments > Partnership Assessment Form > Residential  Information Section
  2. Fill out the fields in the following categories as appropriate to the client within the past 12 months:
  • General Living Arrangement
  • Shelter/Homeless
  • Supervised Placement
  • Hospital
  • Residential Program
  • Justice Placement

Note: The number of days in the 'Number of Occurrences' fields must total 365 days.
