MHSA - Partnership Assessment Form - Administrative Information Section - Cal-PM

Enter administrative information.


  1. Go to: Avatar PM > Client Management > California Data Reporting > MHSA Assessments > Partnership Assessment Form > Administrative Information Section
  2. In the Staff Member field, enter the staff member name click the Search button, and select the corresponding entry.
  3. The Partnership Service Coordinator ID field shows the practitioner’s partnership service coordinator ID. This information populates from the Practitioner Enrollment form (Partnership Service Coordinator ID (MHSA) field).
  4. In the In which programs is the partner CURRENTLY involved field, select the programs. This field is enabled if the client's treatment program is associated with an AB2034 treatment program (Program Maintenance form).
  5. In the In which additional programs is the partner CURRENTLY involved field, select the programs.
  6. In the Gender field, select the client’s gender.
  7. In the Ethnicity field, select the client’s ethnicity.
  8. In the Veteran field, select the client's veteran status.
  9. In the Insurance at Enrollment field, select the client’s insurance at the time of enrollment.
