Family Registration - Family Members Section - Cal-PM

Enter family member information.


  1. Go to: Avatar PM > Client Management > Family and UMDAP Management > Family Registration > Family Members
  2. Next to the Family Membership Information table, select the Add New Item button.
  3. In the Client ID # field, enter the client name, select the Search button, and select the corresponding entry.
  4. The Family Member Name field displays the client's name.
  5. In the Type of Family Member field, select the family member’s position in the family.
    Note: Selecting a family member type who lives in the household disables the 'Client Address' fields.
  6. In the Relationship To ‘Head Of Household’ field select the family member’s relationship.
  7. In the Start Date of Family Membership field, enter the date the family member joined the family.
  8. In the End Date of Family Membership field, enter the date the family member left the family (if applicable).
  9. In the Sex field, select the family member's gender.
  10. In the Date Of Birth field, enter the family member's birth date.
  11. The Age at Time of Data Entry field displays the family member's age at the time the entry was created.
  12. In the Country of Origin field, select where the family member is from.
  13. In the Ethnic Origin field, select the family member's ethnicity.
  14. In the Client's Address fields, enter the address information.
  15. In the Foster Child field, select whether or not the family member is a foster child.
