Substance Abuse Assessment
Record a client's substance abuse activity and related behaviors.
- Go to: Avatar CWS > Assessments > User Defined Assessments > Substance Abuse Assessment.
- Search for the Client Name/ID in the Select Client screen by searching for their ID number or in Last Name,First Name format.
- If the client has more than one episode, select the appropriate episode.
- The pre-display will appear if a record has been filed for the client and episode. Otherwise, the form will open.
- In the Assessing Clinician field, search for the clinician and select their name.
- In the Assessing Date field, enter the date of assessment by entering it in MM/DD/YYYY format, using the calendar icon, or selecting the T or Y buttons for today or yesterday.
- In the Assessment Time field, enter the assessment time by entering it in HH:MM AM/PM format or by selecting Current Time.
- In the Treated for Substance Abuse Problem in the Past field, select Yes or No.
- If Yes is selected, in the Location and Dates of Treatment field, enter information.
- In the Does Anyone in Your Family Have a Substance Abuse Problem? field, select Yes or No.
- If Yes is selected, enter comments in the Comments field.
- In the Drug Choice 1 field, select a drug from the drop down menu, then complete the following fields:
- Select the amount in the Amount field.
- Select the frequency in the Frequency of Use field.
- In the Route of Administration field, select the route.
- In the Age of First Use field, enter the age.
- In the Date of Last Use field, enter the date by entering it in MM/DD/YYYY format, using the calendar icon, or selecting the T or Y buttons for today or yesterday.
- Repeat these steps for any additional drug entries.
- Complete the following fields on the form as applicable.
- Navigate to the CAGE section and answer the questions.
- Once finished, select Submit.
