Staff Assessment of Resident Mood (PHQ-9-OV)

Create an assessment based on staff observation of a client's mental and emotional state as part of a workflow.

1. The Assessment Date field defaults to today's date. Once filed, this date cannot be changed.
2. The Assessment Practitioner field defaults to the staff member who is associated with the logged-in user.
3. In the Assessment Status field, select whether this assessment is a draft or final version. Once set to Final, the assessment will become view only.
4. In the Select Staff to Notify field, select the staff member to notify upon filing.
5. For fields A. through J., select a frequency for resident behavior within the last two weeks.
6. The Total Score field automatically calculates the resident score based on the answers in fields A. through J. according to the following:
   • 0 = Never or 1 day (not at all)
   • 1 = 2-6 days (several days)
   • 2 = 7-11 days (more than half the days)
   • 3 = 12-14 days (nearly every day)
7. If any score other than "0" has been indicated for field I., select whether or not the appropriate person was notified in the Was responsible staff or provider informed that there is a potential for self harm? field.
