Record the Primary, Secondary, and Tertiary diagnostic assessment for Axis I, II, and III diagnoses.

  • This section is also used to record GAF scores as well as recommendations.
  1. If appropriate, enter the client's primary diagnosis code, click Process Search. Click Ok.
  2. For each diagnosis entered, select the type of diagnosis.
