How to Alias the Vitals Entry Form

Aliasing is the process of collecting or displaying information that is also collected in another form/table. Because the Vitals Entry form is used often, there is a different process to set up aliasing. Here are the steps to create a line on the Vitals Entry form and alias the applicable fields to another assessment.

In order to save the form, a date and time column need to be added and mapped back to the Vitals Entry form (i.e. vitals_date and vitals_time).

Note: Steps 9 – 12 only need to be completed once.

Prerequisites:


  1. Search for Table Definition within the search field of the Forms and Data widget.
  2. Search for the applicable CWS Table (i.e. hch_health_screening) within the Search window. Highlight the applicable table and click Select.
  3. Navigate to the Column Definition section of the form.
  4. Click Add New Item below the table.
  5. Enter the Column Name (e.g. physical_pain).
  6. Enter the Column Description (e.g. On a scale of 1-10 is client experiencing pain).
  7. Observe the Column Label displays text from the Column Description (e.g. on a scale of 1-10 is the client experiencing pain).
  8. Select the Type of Column (e.g. Dictionary – Single Response).
  9. Navigate to the Table Alias section of the form.
  10. Click Add New Item next to the table.
  11. Select the Alias Table Entry Database (e.g. CWS).
  12. Select the Alias Table (e.g. Vital Signs).
  13. Navigate to the Column Mapping section of the form.
  14. Click Add New Item next to the table.
  15. Select the Table Column (e.g. physical_pain).
  16. Select the Alias Table Column (e.g. Pain).
  17. If the Type of Column selected in Step 8 is a dictionary, follow the below steps. If not, proceed to Step 18.
    1. Navigate to the Column Definition section of the form.
    2. Select Yes for Does Dictionary Map to A Different Entity.
    3. Select the Entity Database (e.g. CWS).
    4. Search for the Dictionary Description (e.g. Pain).
  18. Repeat Steps 3 – 8 and Steps 13 – 17 until all applicable columns have been mapped.
