Family Health History

Record a client's family health history. This form supports the entry of SNOMED codes to denote problems reported for family members.

Prerequisites:


  1. Go to: Avatar CWS > Other Chart Entry > Family Health History
  2. In the Select Client screen, enter the client name, and select.
  3. In the Select Family Member field, select Add New, or the family member.
  4. In the First Name field, enter the client's first name.
  5. In the Last Name field, enter the clients last name.
  6. In the Relationship field, select the client's family relationship.
    • If the relationship is not listed, in the Other Relationship field, enter the relationship.
  7. In the Date of Birth field, enter the client's date of birth.
  8. In the Sex field, enter the client's gender.
  9. In the Race field, select the client's race.
  10. In the Ethnicity field, select the client's ethnicity.
  11. In the Additional Information field, select additional client information, as applicable.
  12. If the client has died, in the Cause of Death field, enter the unique search term for the cause, and select.
    • If the cause is not listed, in the Other Cause of Death field, enter the cause.
  13. In the Age at Death field, enter the age.
  14. In the Health Problems To Record field, select Yes if there are problems to record.
  15. Click Print Family Member Health History to generate a report that details information displayed in the Family Member Health History field.
  16. In the Notes field, enter any associated notes.
  17. Click Enter Health History to enter client health information.
    • Click New Row to add a row, or edit an existing row.
      Results: The Family Health History screen displays.
      • In the Problem field, enter the problem, and select from the list.
      • In the Other Problem field, enter the problem, and select.
      • In the Age At Consent field, select the client's age.
      • In the Status field, select the problem's current status.
      • In the Treatment field, select the problem's treatment status.
      • In the Treatment Result field, enter treatment results, if applicable.
      • In the Cured Date field, enter date, if applicable.
      • In the Notes field, enter any associated notes.
      • Click Save.
  18. Select whether or not the family member is currently Living In The Household.
  19. Click Submit.
