Create Independent Group Progress Notes (Group Indiv)
Create independent group progress notes.
- Select the Group Default Notes section.
- In the Date Of Group field, enter the date.
- In the Practitioner field, enter the practitioner name, click Select Client Name/ID Search. Click Ok.
- In the Progress Note For field, select Independent Note.
- In the Group Name Or Number field, enter the group name, click Process Search to select. Click Ok.
- In the User To Send Scratch Note To-Do Item field, select the user.
- In the Note Type field, select the progress note type.
- If appropriate, in the User To send Co-sign To-Do Item To field, select the user to co-sign the To-Do item.
This user will receive a To-Do item to co-sign a progress note for each group member.
The User List Management and Notification Users forms define available users.
Depending on the note type selected, this field may be enabled. -
In the Note field, enter associated progress note text.
- In the Client Who Attended Group field, select all of the clients who attended the group.
- In the Missed Visit Service Code field, select the code.

Click File Note.

A client can be added to a group when a group progress note is entered. The client will be added only for a single session. Group assignments are not affected.
When the Enable Service Entry Restriction for Program field is set to Yes on the myAvatar PM Program Maintenance form, the client cannot be added here if the service is not eligible for the program.
To add a client to a session:
- Click Add Client.
- In the Client To Be Added To Group field, enter the client name, click Process Search to select. Click Ok.
- If the client has multiple episodes, in the Episode Number field, select the client episode.
- A message displays. Click Yes.
To remove a client from a session:
- Click Remove Client From Group.
- In the Removal Selection field, select the client.
- A message displays. Click Yes.
