Activities of Daily Living Assessment

Activities of Daily Living (ADLs) can be tracked using this assessment. This includes tracking walking ability, bathing ability, elimination ability, as well as client risk factors and activities that are fundamental for self-care.

  1. Go to: Avatar CWS > Assessments > Product Assessments > Activities of Daily Living Assessment.
  2. On the Select Client screen, search for and select the desired client.
  3. If the client has multiple episodes, choose the correct episode and select Ok.
  4. Enter the Assessment Date by selecting the T (today's date) or Y (yesterday's date) button or the calendar icon, or by typing the date in MMDDYYYY format.
  5. Enter the Assessment Time in one of these ways: 
    • Select the Current Time button.
      OR
    • Select the time using the hour, minute, and AM/PM arrows. 
      OR
    • Type the time in one of these formats: HH:MM AM, HH:MM PM, or HHMM (military time).
  6. For Assessment Status:
    • Select Draft to keep the assessment in draft form. The assessment can still be edited while in Draft.
      OR
    • Select Final to finalize the assessment.
  7. For the Provider, type the first few letters of the provider's name associated with the assessment, then select from the list.
    Note: The practitioner must be tied to a practitioner role in the system to appear in the selection list.
  8. For Select Staff to Notify, type the first few letters of the staff member's name to notify, then select from the list.
    Note: The individual must be tied to a staff member role in the system to appear in the selection list.
  9. Choose the areas where the client needs assistance, such as walking, dressing, bathing, attending to oral hygiene, and elimination.
  10. If the assessment was not completed for the client, search for and select the reason why in the Reason For Not Administering field.
  11. Enter any associated Comments.
  12. Select Submit.
