About the Problem List
The Problem List in myAvatar is where factors impacting the health and functioning of a client can be recorded for the purpose of providing a comprehensive and accessible list of patient problems in one location. These factors can include, but are not limited to, injuries, illnesses, chronic medical conditions, and diagnoses.
You can use the Problem List to communicate with other healthcare providers a client's issues requiring immediate attention as well as the history of prior items. This is intended to support and facilitate improved coordination of care among practitioners contributing the client's care.
Maintenance of a Problem List is required to meet various Meaningful Use and accreditation body standards.
Available status options and their descriptions:
Active: Issues that impact an individual’s current functioning and which are candidates for treatment.
Inactive: Previously-identified problems not considered active, such as those that were identified via Assessment but determined to not be problems. This is similar to a rule-out diagnosis.
Unresolved: Issues that are current and ongoing but not actively addressed in the current treatment plan. An illness or injury, for example, that the agency is not equipped to treat.
Monitoring: Identified problems that are being tracked but have not been confirmed or refuted. These may be changed to Active, or Inactive depending on the outcome of the monitoring.
Resolved: Previously-identified problems that have been resolved. This may be an illness or injury, or something like homelessness that is no longer a problem.
Unresolved: Similar to Active, but an alternative status for things that are ongoing but may warrant a different “active” designation. An illness or injury, for example, that the agency may not be equipped to treat, but needs to include as an ongoing problem.
Auto Delete From Treatment Plan: This is the out-of-the-box void status, and one that I have ALWAYS inactivated and replaced with something else. But the idea here is that voided statuses are deemed inappropriate/incorrect for the selected client and should be removed from the problem list.
