ARRA Eligible Provider Definition screen

If applicable, click New Row.
In the Eligible Provider field, select the provider.
In the Incentive Program field, select the program.
In the Medicaid State field, select the state for Medicaid programs.
In the Assignment field:
- Select Facility if the provider is employed by a facility.
- Select External if the provider is employed outside a facility.
- This information will not be included in ARRA measures reporting.
In the Participation Year field, select the year.
The Reporting Period field displays the reporting period.
In the Reporting Year field, select the reporting year.
The Date From and Date To fields display the from and through dates for the period.
In the Incentive Amount field, enter the amount.
In the Comments field, enter any comments associated with the provider.
Click Save.
