Transfer client to another program (outpatient) - PM

Transfer a client from one outpatient program to another within an episode of care.

Prerequisite:

• The client must be enrolled in an outpatient treatment program.

1. Go to: Avatar PM > Client Management > Episode Management > Program Transfer (Outpatient).
2. In the Select Client screen, enter the client name or ID, select Search and select the client.
   • If there are multiple outpatient episodes, select the appropriate entry from the list.
3. The Program Transferred From field shows the current program assignment.
4. For the Date Of Transfer, enter the date the client is assigned to the new program.
5. For the Time Of Transfer, enter the program transfer time.
6. The Current Treatment Setting field shows the program treatment setting.
7. For the Program, select the treatment program.
   Note: The Allow Admission To File/Edit Pre-Admits registry setting can affect the Program field.
8. The Treatment Setting, Treatment Service, and RRG fields display information associated with the program.
9. For the Team Assignment, assign the client to a team. This is similar to assigning a team at the time of admission.
10. Select Submit.
