Cal-OMS Administrative Discharge - Cal-PM
Collect information when an administrative discharge is performed for a client enrolled in a California Outcomes Measurement System (Cal-OMS) treatment program.
- Cal-OMS Administrative Discharge is used to discharge a patient when the Standard Discharge data is not available, but a discharge needs to be performed. Discharge records that are coded 4 (Left Before Completion w/ Satisfactory Progress / Administrative [minimum questions]) or 6 (Left Before Completion w/ Unsatisfactory Progress / Administrative [minimum questions]) are considered Administrative Discharges. This means that the discharge data used to calculate client outcomes and program performance measures are not available.
- Administrative Discharges lack the information needed to calculate these measures. DHCS includes the number of administrative discharges in formulas used to calculate outcomes and performance. Large percentages of administrative discharges negatively affect outcomes.
- Once a client has been given an administrative discharge, they are available to be re-admitted to a Cal-OMS program through the Cal-OMS Admission form.
- The client must have been assigned to a Cal-OMS program through the Cal-OMS Admission form.
- A client must be discharged from a treatment program in either the Discharge, or Discharge (Outpatient) form.
- Either the Enable LA County Reporting Requirements, or the Enable Non LA County Requirements registry setting must be enabled to process Cal-OMS data collection and submission.
- Go to: Avatar PM > Client Management > Client Information > Cal-OMS Administrative Discharge
- In the Select Client screen, search for and select the client by name or ID.
- The Episode Selection pre-display shows if the client has multiple episodes. Select an episode, click OK.
- In the Discharge Status field, select the client's discharge status.
- In the Disability field, select the client disability.
- In the Current First Name field, enter the client’s first name if different from the birth name.
- Enter "99904" if the client is unable to provide an answer.
- In the Current Last Name field, enter the client’s last name if different from the birth name.
- Enter "99904" if the client is unable to provide an answer.
- In the Primary Drug (Code) field, select the client’s primary drug of use.
- If a value implying a selection of 'other' is chosen, enter the name of the client’s primary drug in the Primary Drug (Other) field.
- In the Primary Drug Frequency field, enter the drug use frequency.
- In the Primary Drug Route of Administration field, select the client’s primary drug route.
- In the Primary Drug Age of First Use field, enter the client’s age of first drug use.
- In the Pregnant At Any Time During Treatment field, select Yes if the client was pregnant at any time during the admission.
- In the ZIP Code At Current Residence field, enter the client's ZIP code.
- Enter "00000" to indicate that the client is homeless.
- Enter "XXXXX" to indicate that the client declines to state their ZIP code.
- Enter "ZZZZZ" to indicate that the client is unable to answer.
- Click Submit.
