Working Diagnosis - PM

Record ruled out, provisional, and accepted diagnoses. Diagnoses filed in the Diagnosis form cannot be opened in the Working Diagnosis form, and vice versa.

  1. Go to: Avatar PM > Client Management > Client Information
  2. In the Select Client screen, enter the client name or ID, and select.
    If the client has multiple episodes, the Episode Information pre-display shows. Select the episode and click OK.
    If the client has previous working diagnosis records, select the record. Click Edit to change information, or click Add to create a new record.
  3. In the Select Episode To Default Diagnosis Information From and Select Diagnosis Entry To Default Information From fields, select a previously filed episode and diagnosis to use as defaults for diagnosis information.  
    Note: The Select Episode and Select Diagnosis fields display if the Default Previously Filed Diagnosis Info registry setting is enabled.
  4. In the Type of Diagnosis, Remarks, and Working Diagnosis fields for Axis I through Axis III, enter the appropriate information. 
    Note: The Remarks fields display if the 'Add Axis I, II and II Remarks' registry setting is enabled.
  5. When finished, click Submit.
