Referral Detail Screen - PM

View details of the referral.


  1. In the Client Search field, enter the client name and select.
    • If the client is in myAvatar:
      • Select the client and click Select to associate the referral with the client.
      • In the Client Information section, the Client ID field populates with the client's ID.
      • Click Admit to open the Admission form for the client. Client demographic information populates and can be edited.
    • If the client is not in myAvatar:
      • Click Admit to open the Admission form, where the client can be entered into myAvatar.
  2. Select the applicable episode in the Episode field, if there are multiple episodes on file.
  3. In the Provider Referred To field, enter the provider to whom the client is being referred.
  4. Click Clear to clear the Client ID field and remove the client association with the referral.
  5. The Referring Provider section displays the referring provider demographic information.
  6. The Status/Notifications section displays the referral status and notification history.
    • Click the icon to open a dialog that displays notes associated with the referral.
  7. The View CCD Section displays details of the CCD.
