Outside Providers - PM

Maintain a record of providers outside of the facility that provide client services.

  • This form is for information-only and can be modified using the modeling functionality of RADplus Utilities.
  1. Go to Avatar PM > Client Management > Client Information > Outside Providers.
  2. In the Select Client screen, enter the client name or ID, search for and select the client.
    • If the client has multiple episodes, the Episode Information pre-display shows. Choose the episode and select OK.
  3. If the client has previous outside provider records, select the record then select Edit to change information or select Delete to remove the provider.
    • Select Add to create a new record.
  4. In the Outside Provider Type field, select the type of outside provider.
  5. In the Outside Provider Agency field, enter the provider's agency name.
  6. In the Outside Provider Name field, enter the provider's name.
  7. In the Outside Provider Title field, enter the provider's title.
  8. In the Outside Provider ID field, enter the provider's ID.
  9. In the Outside Provider Phone field, enter the provider's phone number.
  10. In the Outside Provider Date Last Seen field, enter the date the provider was last seen, or select from the drop down calendar.
  11. In the Outside Provider Seen How Often field, enter the recurrence of visits with this provider (weekly, monthly, etc.).
  12. Select Submit.
