Enter client charges - PM

Enter services (charges) for a client. Entering a service posts the service fee to the client ledger, and adds the services to the client’s treatment history.

Note: The Cost Of Service field can be edited in the Client Charge Input (Charge Fee Access) and the Client Charge Input (Charge Fee Access and Diagnosis Entry) form.

Prerequisites:

  1. Go to Avatar PM > Services > Ancillary/Ambulatory Services > Client Charge Input.
  2. In the Date Of Service field, enter the service date in MMDDYYYY format, using the calendar icon, or by selecting the T (today) or Y (yesterday) buttons.
  3. In the Service Start Time field, enter the start time using the HH:MM AM/PM format, or by using the Current Time button. 
  4. In the Service End Time field, enter the end time using the HH:MM AM/PM format, or by using the Current Time button. 
  5. In the Client ID field, enter the client name or ID, and select.
  6. In the Episode Number field, select the client episode.
  7. In the Program field, select the client program. 
  8. In the Service Code field, search for the service code name or number, and select.
    Note: The service must be required by a client or a client and practitioner (Service Required By field on the Service Codes form).
  9. In the Original Scheduled Service field, search for the original scheduled service, if applicable. 
  10. In the Modifiers field, enter the appropriate modifier(s). The modifier should stay in the field when you tab away. Multiple modifiers can be separated by a comma or a colon. Modifiers in this form will override those normally generated by a service fee cross reference. 
  11. In the Practitioner field, search for the practitioner name or ID, and select.
  12. Click Display Managed Care Authorization Data to display managed care authorizations for the client, service code, and date of service. The data will open in a new window. 
  13. In the Location field, select the service location from the dropdown menu. 
  14. In the Duration (Minutes) field, enter the service duration.
  15. In the Co-Practitioner field, enter the practitioner name or ID, and select.
  16. The Cost of Service displays the service fee, setup in the Service Codes and Service Fee/Cross Reference Maintenance forms.
  17. In the Referring Practitioner field, search for and select the referring practitioner. 
  18. In the Referring Provider field, search for and select the referring provider. 
  19. In the Date of Accident (2300-DTP-03) field, enter the date in MMDDYYYY format, using the calendar icon, or by selecting the T (today) or Y (yesterday) buttons. 
  20. In the Related Causes Information (2300-CLM-11) field, select the related cause(s). 
  21. In the Form Locator 10 (Auto Accident, Place) field, select a form in the dropdown menu.
  22. Select an Add-On Service from the dropdown menu. 
  23. In the Add-On Duration field, enter the duration of the add-on service in minutes. 
  24. In the Add-On Notes field, enter any necessary clinical notes for the add-on service. 
  25. Select the Save Add-On Service button to save the information about the add-on service. 
  26. To edit or remove a service, in the Select Add-On Service Entry to Edit/Remove dropdown, select the service to edit or remove. 
  27. If removing a service, select the Remove Add-On Service button to remove the service. 
  28. Enter the duration of the co-practitioner's time in the Co-Practitioner Duration (Minutes) field. 
  29. Select Submit.
