Supplemental Diagnosis Information

Enter additional diagnosis information for a member.

• This form can be modeled through RADplus Utilities.

1. In the Select Client screen search for and select the client by name or ID.

• If the member has previous supplemental diagnosis records, the pre-display shows.
• Select a row, click Edit.
• Or, click Add to create a record.

2. In the Type Of Diagnosis field, select the diagnosis type.

• This field is maintained in the User Defined Member File (Dictionary Update form).

3. In the Date Of Diagnosis field, enter the diagnosis date.

• Diagnosis axis and maintained in the Diagnosis Table Maintenance form (RADplus Utilities).

4. In the 'Diagnosis Axis' fields, enter the diagnosis code, click Process Search to select. Click Ok.

5. In the 'Axis IV' fields, select Yes if the diagnosis applies to the type of environment.

6. In the Axis V Current GAF Rating field, select the description of the member's current GAF rating (Global Assessment Functioning).

7. In the Axis V Highest GAF Last 12 Months field, select the description of the member's highest GAF.

8. In the Axis V Lowest GAF Past Year field, select the description of the member's lowest GAF.

9. In the Diagnosing Comments field, enter comments associated with the diagnosis.

10. Click Submit.
