Click a row to select:

Columns

• Code Type
  Enter the service code type:
  'C' - CPT code
  'R' - revenue code

• R - revenue code

• Procedure Code
  Enter the service code.

• Requested Units
  Enter the number of units.

• Units Authorized
  Enter the authorized number of units.

• Estimated Liability Code
  Displays the total liability codes, for the authorized service codes.
  Estimated liability is calculated by multiplying the number of units authorized, by the service cost.

Fields

• Calculate Estimated Liability
  Click to calculate the estimated liability.

• Remove Selected Rows
  Click to remove a service row form the service authorization.

• Enable Col Resize
  Click to allow a column to be resized.
  Drag the column, when finished click Enable Col Resize.

• Add Row
  Click to add services to the authorization.

• Select All
  Select all rows.

• Deselect All
  Remove the row selection.
  When finished, click OK.
