Provider Assignment

Associate a member with a contracting provider, and funding source, for facilities with CareLink enabled.

Note: This functionality is not available in conjunction with ProviderConnect.

  1. Go to: myAvatar MSO > Provider Management > Provider Assignment.

  2. In the Add / Remove field:

  • Select Add to create a provider assignment.

  • Select Remove to delete a provider assignment (Assignments field).

  1. In the Start Date field, enter the provider assignment start date.

  2. In the Member Selection field, enter the member name, and select.

  3. In the Primary Contracting Provider field, search for and select the contracting provider.

  4. In the Funding Source field, enter the funding source name, and select.

  5. In the Assignments field, select the provider assignment to remove.

  6. Click File.

  7. Click Display Providers to generate a report that details member associations entered in this form.
