Use the Certifying Physician Statement tab to configure statements for certification and recertification based on effective date and patient class.

Option Description and Text

Patient Class

HomeHealth

Select this option to configure certification and recertification statements for home health patients.

Hospice Select this option to configure certification and recertification statements for hospice patients.

Certification tab

Effective From Specify the effective date for the certification statement. By default, this date is set to the date that you installed the current release of Netsmart Homecare.
Generic Text

This text is printed on the generic form, directly above the physician signature. This form is used if you selected G in the Forms for Certs/Recerts box in Administration > General > Patient Classes for the corresponding patient class. You can enter up to 2000 characters.

Netsmart delivers the following default text for certifications for all patient classes:

I certify that this patient is confined to his/her home, and needs intermittent skilled nursing care, physical therapy, and/or speech therapy, or continues to need occupational therapy. I certify that this patient is under my care, and I have authorized the services on this plan of care, and that I will periodically review the plan of care. I also certify that my review includes the addendum(s) attached to this form.

485 Text

This text is printed in box 26 of the 485 form. This form is used if you selected 5 in the Forms for Certs/Recerts box in Administration > General > Patient Classes for the corresponding patient class. You can enter up to 550 characters.

Netsmart delivers the following default text for certifications for all patient classes

I certify that this patient is confined to his/her home, and needs intermittent skilled nursing care, physical therapy, and/or speech therapy, or continues to need occupational therapy. I certify that this patient is under my care, and I have authorized the services on this plan of care, and that I will periodically review the plan of care. I also certify that my review includes the addendum(s) attached to this form.

To change the certification text and use the updated certification statement from a particular date onward, set Effective From to the date to start using the new statement. Edit the statements as needed and click OK. Certification orders created on or after the effective date use the updated statement text.

Note: The default statements are written primarily for the HomeHealth patient class. If you are using the Hospice or Other patient classes, your organization should edit the statements per the needs of the organization. Alternately, to continue using the statements defined in Administration > Configuration > Business Units > Settings > Hospice Orders, when you first install or upgrade Netsmart Homecare, go to Administration > Configuration > Organization > Basic > Settings > Certifying Physician Statement, select Hospice or Other and set Effective Date on each certification and recertification statement to a date in the future. Until that date is reached, the prior functionality is used.

Recertification tab

For recertification orders, the estimated length of service is included in the statement. The estimated length of service value is filled in from Patient > Clinical > Visit Frequency > Estimated Length of Service.

Effective From

A variable (%P%) provides a placeholder in the statement for the estimated length of service, which is filled in on the form from the value entered in Patient > Clinical > Visit Frequency > Estimated Length of Service. You cannot remove this variable from the statement. You can enter up to 2000 characters.

Home Health patient class

Netsmart delivers the following default text for recertifications for the Home Health patient class:

I certify that this patient is confined to his/her home, and needs intermittent skilled nursing care, physical therapy, and/or speech therapy, or continues to need occupational therapy. I certify that this patient is under my care, and I have authorized the services on this plan of care, and that I will periodically review the plan of care. I certify that this patient will need home health services for an estimated length of %P%. I also certify that my review includes the addendum(s) attached to this form.

A variable (%P%) provides a placeholder in the statement for the estimated length of service for the Home Health patient class, which is filled in on the form from the value entered in Patient > Clinical > Visit Frequency > Estimated Length of Service. You cannot remove this variable from the statement. The statement is required for Home Health recertifications.

Hospice and Other patient classes

Netsmart delivers the following default text for recertifications for the Hospice and Other patient classes:

Default Value

Default Value is displayed in black text. If your organization has not modified the text in Generic Text or 485 Text, the heading (Generic Text or 485 Text) is displayed in black to indicate that the corresponding box contains the default text.

Changed Value

Changed Value is displayed in blue text. If your organization has modified the text in Generic Text or 485 Text, the heading (Generic Text or 485 Text) is displayed in blue to indicate that the corresponding box contains text that was modified from the default value.

Restore Default

If you are editing text in one of the statement text boxes and have not clicked OK, you can click the Restore Default icon to undo your changes and revert to the default version of the statement.
