Integration between Netsmart Referral Management and Netsmart Homecare

By interfacing with Netsmart Referral Management, Netsmart Homecare can make the job of handling referrals easier, faster, and less error-prone.

The interaction between Netsmart Referral Management and Netsmart Homecare application proceeds according to the following process flow:

  1. A facility's discharge planner uses Netsmart Referral Management to send a patient referral to Netsmart Homecare via an HL7 message.
  2. After an intake specialist approves the HL7 message, Netsmart Homecare creates a new prospective patient based on the patient referral.
  3. Clinicians viewing the patient's record in Netsmart Homecare can follow a link to access Netsmart Referral Management and view the patient's referral information.
  4. Once the agency admits a patient, Netsmart Homecare automatically sends the Start of Care date to Netsmart Care Management.

The integration between Netsmart Referral Management and Netsmart Homecare application provides an agency with the following advantages:
