Patients with Unresolved Diagnoses reported situations

Prerequisites: 

  • To access this report, you need to have the appropriate privilege granted by the Netsmart Homecare administrator. 
  • Ensure that the Reporting Core Components is installed.

Unresolved diagnoses are reported in the following situations:

Situation Explanation

Free-text diagnoses/procedures are not converted

Free-text diagnoses are added. Such diagnoses must be converted to the diagnoses with the valid codes.

Free-text inpatient diagnoses are not converted

Free-text inpatient diagnoses are added. Such diagnoses must be converted to the diagnoses with the valid codes. Inpatient free-text diagnoses should be also converted directly in an assessment. See Inpatient Diagnoses Workflows.

ICD-9 diagnoses/procedures are not converted into ICD-10

Number of patient's active ICD-9 diagnoses or procedures is greater than the number of ICD-10 diagnoses or procedures, which means that potentially not all diagnoses are converted to ICD-10.

Primary diagnosis is not ICD-10

You may have converted all ICD-9 diagnoses into ICD-10 codes, but the diagnosis selected as primary in the Admissions & Status window is not ICD-10 after the effective date for ICD-10.

Patient diagnoses group is not valid

When diagnoses group has no primary diagnoses within the group date range. This may happen if you end the diagnosis assigned to a group in Field Mode. Diagnoses groups are not validated in Field Mode, so after synchronization, you need to correct groups in Host Mode.

Diagnosis code entered in Field Mode is not auto-resolved

When the code that is entered in Field Mode does not meet criteria for automatic code resolution:

  • A code does not exists or has multiple descriptions.
  • The start date of the entered diagnosis code is prior to the start date of the last diagnosis in the group.
  • A diagnosis in the last diagnoses group is discontinued.
  • A diagnosis is defined incorrectly. For example, ICD-9 code that starts with "E" and set as primary, or the primary diagnosis has OASIS/Reference as special indicator.)

Inpatient diagnosis code entered in Field Mode is not auto-resolved

When the code for the inpatient diagnosis that is entered in Field Mode does not meet criteria for automatic code resolution. For example, a code does not exists or has multiple descriptions.

Procedure code entered in Field Mode is not auto-resolved

When the procedure code that is entered in Field Mode does not does not exists or has multiple descriptions.
