Patient Name	First and last name of the patient entered in Patient>General>Basic.
Patient Code	Patient code entered in Patient>General>Basic.
Date of Birth	Patient's date of birth entered in Patient>General>Basic.
Date Completed	Date when a patient was discharged.
Team	Patient's team entered in Patient>General>Admissions & Status.
Marital Status	Marital status of a patient entered in Patient>General>Basic.
Age	Age of a patient entered in Patient>General>Basic.
Advance Directives	Advanced directives for a patient entered in Patient>Clinical>General Clinical>Advance Directives.
County	Name of the county where a patient lives entered in Patient>General>Demographics.
Ethnicity	Ethnicity of a patient entered in Patient>General>Demographics.
Living Status	Living status of a patient added in Patient>General>Demographics.
Religion	Religion of a patient entered in Patient>General>Demographics.
Date of Referral	Date when a patient was referred the last time as specified in Patient>General>Admissions & Status.
Date of Admission	Date when a patient was admitted as specified in Patient>General>Admissions & Status.
Discharge Reason	Reason why a patient was discharged as specified in Patient>General>Admissions & Status.
Mental Status at Discharge/Transfer	Mental status of a patient at the time of discharge or transfer as specified in Patient>General>Admissions & Status.
Final Date of Service	Date when the final service to the patient was provided.
Services Provided	List of the services provided for a patient.
No. Vis	Number of visits provided for a patient.
LOS	Patient's length of stay.
Days/Hours	Number of days or hours spent for a certain service.
Goals/Expected Health Status Evaluation	Description and dates of the expected health status evaluation and potential results of treatment specified in Patient>Clinical>Care Plans.
Health Status Evaluation	Health status evaluation of treatment as specified in Patient>Clinical>Care Plans.
Ended	Date of the goal termination.
