Quality Measures window
Path: Patient>Clinical>Quality Measures
Prerequisite: To access the Quality Measures window, you need to have the appropriate privileges granted by the Netsmart Homecare administrator.
The Quality Measures window contains the following sections:
- Initial
- Within 24 hrs
- Within 48 hrs
Initial
Use this section to document the first patient's comfort level assessment (either during the initial assessment or during the pain episode). Complete the following fields:

Date |
Specify the date of the initial comfort level assessment. |

Initial Assessment |
Select this check box if you perform the first comfort level assessment after the patient's admission. |

Pain Episode |
Select this check box if you perform the comfort level assessment on the subsequent episode of pain. |

Patient is able to self-report |
Check patient's ability to answer the comfort level question, and then select Yes if a patient can self-report, No if a patient cannot. |

Reason |
If a patient cannot self-report, select the reason why. |
|
Are you uncomfortable because of pain? |
If a patient can self-report, ask the comfort question, and then select Yes if a patient experiences pain, No if a patient does not. |
| Connect to Visit/Provided by |
This field shows whether the selected comfort level record is assigned to a visit or to a resource. If it is assigned, then the corresponding information appears in this field. If it is not, No Visit or Staff assigned to this record is displayed. |
Within 24 hrs
Starting from Netsmart Homecare 8.1, this section is not active for documenting patient comfort level. The information entered in the previous versions of Netsmart Homecare is shown but you cannot edit it.
Within 48 hrs
If Yes is selected for the Are you uncomfortable because of pain? question, this section becomes active. Use it to document the patient's comfort level assessment in 72 hours after the Initial assessment. Complete the following fields:

Check patient's ability to answer the comfort level question, and then select Yes if a patient can self-report, No if a patient cannot. |

Reason |
If a patient cannot self-report, enter the reason why. Xxxx Note: You can select the reason from the drop-down list, edit it as appropriate, or select Other, and then type the reason manually. |

Was your pain brought to a comfortable level? |
If a patient experienced pain during the Initial assessment, you must reassess the comfort level in 48 hours. Ask the comfort question, and then select No if a patient still experiences pain, Yes if the patient's pain was eliminated within 48 hours. Therefore, you document the patient's comfort level within 72 hours after the Initial assessment, which is required. |
