My Patients - Care Team tab

Path: Netsmart Homecare > My Day

You can view and print an Care Team report using this tab.

The Care Team tab contains the following columns:

Section Description

Unresolved Diagnoses

The warning icon indicates that the patient has some unresolved diagnoses. 

Patient Code

Shows the code of a patient. The codes are assigned to patients in Patient>General>Basic.

Patient

Patient's names as defined in Patient>General>Basic.

Note: You can create a task by right-clicking patient name and selecting the Create Task option if have full access privilege to the Alerts and Tasks/My Tasks window.

  • To open patient's information, right-click the patient name and select the Open Patient option.
  • To view medications list, right-click the patient name area and select the View Medications List option.

Status

Current patient's status.

Patient Phone

Phone number of a patient.

Physician

Name of the physician (Patient>General>Admissions & Status).

Physician Phone

Work phone number of the physician (Resource>General>Address & Phone).

Emergency Contact

Emergency contact information as specified in Patient>General>Family & Friends.

Emergency Phone

Phone number of the emergency contact (Resource>General>Address & Phone).

Admission

Patient's latest admission record.

Exit

Indicates the patient's discharge or date of death if the patient is inactive.

City

City where the patient lives.

Date of Birth

Patient's birth date.

Zip

Patient's zip code.

Master ID

Patient's master ID if applicable.

Business Unit

Name of the Business Unit to which a patient belongs.

 

You can print information shown in the My Patients - Care Team section using the Print button located in the top right corner of this section. The detailed report will be printed on a separate page.

Click the Customize button in the top right corner of the My Patients section to define setup options to be displayed in the Care Team tab in the Customize View: My Patients - Care Team window.
